Breast reconstruction has several phases and
are affected by various factors

First of all, the operation’s size is very important. The
reconstructive procedures of partial breast tissue removal
(Lumpectomy – breast protective surgery) and total
breast tissue removal (total mastectomy) are totally
different.

It is necessary to protect the veins during the removal of
lymph glands on the armpit (axillary abortion). If the veins
have been protected, the reconstruction options will be
highly increased and it allows the transplanting of tissues
from the back muscle-skin (latissimus dorsi muscle-skin
phleb) or tissue transplantation by microsurgery from other
parts of the body.

In cases where the tummy muscle (rectus abdominis) will be
used for the reconstruction, extra additions will be made to
the veins and the success chance will be increased.

Previous chemotherapy (CT) or radiotherapy (RT)
candidates that have used this treatments can also have
affected results. In cases of radiotherapy, the reconstruction
with silicone breast implant will not be used. In these cases
the tissue which should cover the implant have been
weakened by the radiotherapy and become low-nourished
tissues, therefore the silicone breast implant can come
away. In surgery of this type, tissue will be used from areas
of the body furthest away from radiotherapy treated areas.

In cases of healthy breasts, the breast will be reduced,
lifted or augmented for equalizing with the reconstructed
breast.

Back muscle (latissimus dorsi): If the volume needed is not
too much and the armpit veins have not been damaged
and the patient accepts the scar on the back, this method
will be used to reconstruct the breast.

Tummy muscle (rectus abdominis): If the patient has a
donor area on her tummy, if lose skin exists, then that can
be taken as tissue, for a fuller breast. At the same time,
the tummy lifting will also be performed. This solution
cannot be used where tummy tissue has had previous
hernia problems or for tight tummies. Another option can
be to use partial hip muscle to reconstruct the breast area
through microsurgery, which may incur a scar from the
donor area and may be a disadvantage.

The Surgery

The operation will generally be performed in two sessions.
During the first session the tissues will be moved, and on
the second session forming, creating the breast nipple
and procedures for the opposite breast will be performed.
Additional sessions may be needed for retouching.
